Can someone attach me Schedule A form for me. The june 2017 form is not valid in our case i think.

this form is January 2017 issued.

Mine didn't generate a bar code, but it said validated in a little box on the upper right of the first page. I did mine before the new forms came out though.

I have a question regarding Schedule A form:
My given name is XXX YYY and familly name is Blank. So according to the guide if Family name is blank then write Given Name in family name field. But when I enter Given name in Family name, after clicking on "Validate Button" I am getting an error that Given name is missing.

Someone please help? Should i write Not applicable in the given name field?

Does this mean that I need one for like all her brothers and sisters that are over the age of 18? what about her mom?

No, you do not need PCC for applicant(s) brothers, sisters or mom!

Its only for the applicant(s) family member that are included in the application for permanent residence. If you are applying for your spouse only, send only her PCC.

Today i don't see applicants that received their AORs today.

This is exactly what I am confused about. That June 2017 form is only for kit 5289 and 5772. So which one we should use. Dec 2012 or June 2017 ?

Use the one that comes with the kit .. not from a drop down list ..

Get the correct forms from here .. dont go searching for forms else where on google or the cic website ..

A lot of people are confusing others by bringing up forms that relate to different kits but not to spouse sponsorship ..

I did not acknowledge that we have to fill new schedule A form and I submitted the old version on gc key. Is there any other way to submit Schedule A latest version other than gc key?
