I have a question, What can be a legal document for name change after marriage??

I am from India and in my country, marriage certificate along with old identification document is enough to prove the name change. Based on this, we get the new identification document with new name.

Please help with the legal document accepted by CIC for this purpose as they don't mention enough information on the website. Thanks!!!

Could anyone please help me with the concern.. I need to work on this document. Thanks!!!

I have a similar question too.. Has anyone faced this scenario and what is the solution to this? The help icon says this "You indicated you have another name. You must provide information on the use of this other name (such as a letter of explanation, legal documents of a name change, etc".

Please help.

Please do affidavit on 20 RS of stamp paper , that will be enough.

I have done the same and submitted my application. Format for that affidavit will be easily available on the internet

you need to make a Name Change Affidavit Document.

you can either go to a lawyer and get it done or visit the nearest notary public and get it notarized.

In some countries, this is already stated on your marriage cerificate.
The last name that was agreed to use after marriage. I had such case, so I was just uploading my marriage certificate again.

Don't worry friend..

Just input the old & new names in the name fields in your online application.
There is a column for the reason of name change in online form (i.e. name change after marriage)

All u need to do is, just attach a marriage certificate which is issued by an appropriate authority in your state/region, and, of course, is in English.
If not in English, then u have to get it translated by appropriate authority.

I am also going to do the same in my file. Just attaching my marriage certificate for legal name change of my wife. she also has her NEW name after marriage in her passport.

In my case, my wife's name is not changed. Her surname in Passport which is issued recently still has her old surname. In my passport, I have her surname as my surname. She has some other documents like Driving License, Bank accounts with my surname which is my concern.

In this situation, do I still need an affidavit of name change? I ask because she hasn't really changed her surname as her passport still has her old surname. But she has used my surname in some cases. So basically she continues to use both the surnames.

An affidavit (preferably notarised) would be required in your case, primarily stating that Name A and B refer to the same person.

In the Marriage certificate, my wife's old surname (before marriage) is not mentioned anywhere. I don't think that I can use Marriage Certificate for Name change. I'll need to have an affidavit for the same. Please suggest?
