Confusion re: request to fill out IMM5562E Travel History form

Hi everyone,

I am applying for Canadian PR under the CEC. Today I received a request to supply additional information. Here is the text of the request which I have edited to remove identifying details:

<UCI Number> - Principal Applicant's Name (me):
 IMM5562: Supplementary Information - Your Travels
 Please use the following link to provide details of your travel history, include day, month, and year as well as duration of time.
<UCI Number> - Spouse's Name (my partner):
 IMM5562: Supplementary Information - Your Travels
 Please use the following link to provide details of your travel history, include day, month, and year as well as duration of time.

The link takes me to the standard IMM5562E PDF form, which I expected. However there are some discrepancies between the instructions they gave me, and the form

1- The message I received specifies: include day, month, and year as well as duration of time but the form itself says only to include the month, year and duration- no day. Which is correct?
2- The form has a section for myself, the principal applicant, and my common law spouse. That's fine. But the message seems to say I should fill out two separate forms, one for me and one for my spouse? To make it even more confusing, two spaces have appeared in *my* section of the application to upload 2 new forms. But there are no spaces in my partner's section, even though one of them should be for him?

Not sure what to do- should I fill out one copy of the form (with both of our details) and upload a duplicate in the second slot? Or fill out one for me, and one for my spouse, even though the instructions in the IMM5562E form clearly state it should only be filled out by the principal applicant.

Any help is appreciated- I have just 7 days to submit the forms and I'm really confused about what to do.

My letter states :

 Please provide a new Travel History that will include the exact time you spent in the United Arab Emirates for the following dates:
 OCTOBER 2010 TO NOVEMBER 2016


I think I only need to enter mine and not of my dependents..

If I were you, I would complete the form as much as it allows, make another table in LOE, add everything they are asking, merge both, upload both form and LOE.. also, mention in LOE that how form doesn't allow you to input days

Thanks for the reply. I ended up calling CIC and that is pretty much what they told me to do. They also agreed that someone on the other end had made a mistake and my confusion was understandable. I ended up filling out the form as per the instructions in the form itself (including both my and my spouse's travel history). In the second slot that opened up, I uploaded a LOE explaining that I filled out the form in accordance with the requirements laid out in the form itself. Hopefully that will be okay!

You might want to include the conversation with CIC in LOE if you haven't submitted the form yet otherwise no problem.

Thanks for the reply. I ended up calling CIC and that is pretty much what they told me to do. They also agreed that someone on the other end had made a mistake and my confusion was understandable. I ended up filling out the form as per the instructions in the form itself (including both my and my spouse's travel history). What was the outcome? I have the same situation. In the letter request letter they have also asked me for certain period residency detail, occupation and travel and at the bottom there is a link for form. That form is only for travel history and not for personal history like residency and occupation. Can you please guide me on this. Thanks
