IMM5669 question number 8 and 12

Hi any one can help me out to fill this question?

I have sent all my docs some time back and now Colombo immigration office had sent me a mail stating that , there is some mistake in IMM5669'.
below is the statement as it is.

""Please complete question 8 (Personal History) and question 12 (Address History) of your schedule A (IMM5669) so that there are no discrepancies in the information relating to your places of residence as your current Schedule A provides conflicting information as to your residence This must be received at this office by: 10/03/2015""

in question number 8; I have provided address details related to working places ; eg: 'during that time I was working in Dubai. so I provided address detail in Dubai.

in Question 12; I have provided address details related to my residence. which means i have provided an address in Sharja for the same period

where I have gone wrong ?

how should I complete these two questions ?

Oh My God !! You have exactly thought in the way I did : And this kind of sets us in the same boat

I received MR & requests for other documents ( PCC etc) and have been asked to fill up Schedule A of form IMM 5669 , points 8 and points 12 AGAIN.
Now , I realized that the addresses I had used for point 8 and point 12 are different .. the reason is - I thought point 8 (Personal History) needs details or work location and point 12(Addresses) only the addresses you have lived.... as a result my point 8 and point12 showed up different addresses.
Do you think this is the reason why they have asked me to resend the IMM 5669 updating points 8 & 12 ?

Any help is highly appreciated as I have to send the documents updated asap.

Can some one please help clarify this ?

Many of us have lived and worked at different cities/towns in the USA, as towns are far but commutable.
Shall town/city #8 under 5669 (Personal History) to match the where you lived #12 (Address History) ?

HI Andy,

still I'm confused how to complete the data.
the only option left is to write back to visa office.
I'll keep you posted if I hear from them.

meanwhile really appreciated if anyone can advise how to complete these two questions correctly.
