There is question :

"Has (My Name) ever been refused a visa or permit, denied entry or ordered to leave Canada or any other country ? "

Actually I got a 221 G white form while I went for H1b visa stamping at USA consulate in Mumbai in 2008 , any one faced such situation ?

What should I answer to this Question - Yes or NO

Yes, that's a Visa denial, unless it was cleared later and you did receive the Visa.
