My dad has a PR card, and he returned to our home country 2 years ago. His health card has expired already. I wonder if he can apply for an Ontario health card when he comes back to Canada and get OHIP covered immediately.

Did you go online and check what the Ministry says ?

https://www.ontario.ca/page/health-card-renewal

If he can’t do it online then he goes into an OHIP office with the required documents

FYI
You may be required to provide additional documents to confirm Ontario is your primary place of residence if your health card is already expired and/or you have been out of the province for more than 7 months in the last 12-month period.”

He can reapply but only if he is planning on making Ontario his permanent home and will be in Canada for 5 out of the next 6 months and 6 out of the next 12 months. Based on your previous posts he would not qualify for the 5 month in a year residency requirement and needs to meet the 1st year residency requirement again
