Canadian citizen getting married in India. So, how can I bring wedding jewelry to Canada? Do I need to declare anything in any form? Where can I get the form?

I assume that you didn't leave Canada for a while (non-resident) is just relocating back. If so, that's a different process/policy.
Moving or returning to Canada

If you are bring jewelry to Canada so you need to declare them and likely if the value is over the normal personal exemption ($800 for 48hr+ trip), you will need to pay duty and taxes on them.
(doesn't matter if you bought them or you got them as a gift)
