Hi My father is on visitor visa and we have applied for his extension. He got the medical request today Nov 7th, but he had booked return ticket for Dec 5th, which is just before 30 days for submission of medical report. Can he leave without doing the medical, or he has to do the medical?

He has to do it since he has benefited from extra time in Canada. If he doesn’t he will have problems visiting Canada in the futures

Medicals are valid for a year. He should reestablish ties to his home country before applying for a supervisa given his recent visit to Canada.

His extension got approved today.
Stating this:
Please note that as per the Immigration and Refugee Protection Regulations (IRPR), a valid
medical examination is required if you are seeking to work in certain occupations in which the
protection of public health is essential. Please review the following page to find out more about
such occupations:
If you do not have valid medical results you may have received condition 15 or 16 printed on your
permit stating “Not authorized to work in 1) child care, 2) primary or secondary school teaching, 3)
health services field occupations (condition 15), 4) agricultural occupations (condition 16)”. If
you wish to remove these conditions on your Permit, you will need to complete a medical
examination and then, you must submit a new application. You also need a valid medical exam if
you are coming to Canada for more than 6 months and you lived in one or more of these
for at least 6 months in a row within the last year.

Does he still need a medical? He doesn't intend to work

Yes. He is not legally able to work as a visitor so irrelevant.
