We did not receive the visitor record for my US born child when we landed in Canada. There were no stamps put in his US passport as well. When we tried to apply for OHIP, Service Ontario is requesting for visitor record for my child. What needs to be done in this scenario - or if some has already faced the same, please share your thoughts.

You would be speaking to CBSA, not IRCC. It is pretty much your only option at this point. IRCC will not issue a Visitor Record unless extending the child's status, which cant be done until closer to expiry.
He, his wife, their first child are canada permanent residents. The second child is a US citizen and they all entered Alberta recently, the second child entering first time into Canada. When went to apply for Alberta Health care, he was asked to provide a visitor record as one of the documents but there is not one.
In Alberta the child should qualify for healthcare but the family must remain in Alberta for the next 6 out of 12 months to qualify to access healthcare in Alberta. Assume the whole family applied for health cards. They needed to ask for a visitor record when entering Canada.
This is the first entry only for the newborn, others already have health cards.They already entered but did not get a visitor record for the newborn. What are their options?

Sounds like they did not make Alberta their permanent home or meet the residency obligation so must reapply. To qualify for provincial healthcare you need to have a valid health card and have met the residency obligation to qualify. To get the visitor record you’ll need to enter Canada again and request the visitor record.
