Please visit a Consultant and check on your eligibility. You would require really good IELTS scores to get the required CRS points and I suggest to get your wife to write IELTS as well. Or you can start to learn French, as that can increase your CRS also. Good luck.

No such thing as immediate immigration. You need to be eligible to submit a profile through Express Entry. So research Express Entry and see if eligible based on your age, education, language skills, work experience and funds. If eligible you want a very high score - Masters, fluent in French and English etc.
There are no immigration programs specifically for disabled individuals. You will need to qualify and apply through a regular economic immigration program like Express Entry.
