That is a very low score. You will add spouse to profile once married as accompanying or non accompanying. As for Alberta, there program has totally changed and is based on a points system. Check the grid and see what your score is but it benefits those educated and working in the province. So now to get nomination, your Alberta score and NOC have to be selected. Having a sister may not be of benefit.
You need to increase your score so max out IELTS and maybe learn French. If you are a doctor then MCC would have completed ECA.

You said you were a doctor by profession. Look at healthcare pathway for the province with job offer but will need license. What is your degree in? Even for EE, you need to increase your score.
So if you cannot increase score, you have to hope that your AINP grid score is high enough and NOC selected without a job offer or being licensed to practice.
