Update name in citizenship certificate

Hi everyone,

I had only last name in my citizenship certificate and I wanted to change it, so I applied for name change in my province and got the name change certificate.

I am filling the name change form, cit001e-2. I am confused and have no clue about point 5 and point 7. Point 5 ask details as per birth certificate, but there is only name in birth certificate, also in the instructions for step 5, it says this is for the first time applicant only. Same goes for point 7.

anyone has filled this form and knows what do about point 5 and point 7, please let me know.

I have not but rather straightforward: put names from your birth certificate. The box in 5 says include other names, including current name. Section 6 says I want a different name to appear.

I see the instructions say first time applicant, I admit that doesn't make sense to me.

Separate point: why do you think you need an updated citizenship cert? You can apply for a passport with new name on basis of your existing certificate and change of name documentation. After that, you will basically never need the citizenship certificate again (of if you do, proceed as above: existing one plus change of name docs), as everyone will accept passport as proof of citizenship and current name.

But if you just want it, that's your choice (and right), of course.
