I am aware that we dont have to do upfront medical. But do we have to do medical later after submitting the application or not? If IME (low risk) is done in 2021 for student visa. Do we still get request for medical after submitting CEC or no?

If you don't have any underlying issue that was mentioned in your previous medicals, I suppose, there's no reason for IRCC to have you take your medicals again. You will directly get as "Medicals Passed" around the time you get your BIL!
