Why medical for a visitor visa ?

Hi I want to get some info regarding I want to apply for my sister family visit viza?she is house wife and husband is business owner,do we need to apply for all or just my sister and kids?any chances they all will get at once or we should apply as without her husband?

I would not apply for kids and sister together just to be safe. The kids would show a strong home tie
