Looking for suggestions to help with a mea culpa letter to attach to a PRV (spousal) application. We made two previous TRV applications (both denied) that failed to disclose an overstay in another country.

I would keep it simple and honest and make sure you take full responsibility for failing to declare the overstay.

Provide the full details of the overstay. Country, dates, length of overstay, reason for overstay, type of visa held before overstay. Also include information such whether a departure or deportation order was issued (or similar depending on the country).

Indicate you failed to declare the overstay in two prior applications. Include the dates of those applications, types of applications (TRV), and outcomes (both refused).

Explain why the overstays were not previously declared. Take full responsibility for this.
