Visa Refusal sept ‘19 Windsor Urgent Help

My Friend visa was rejected because of the reason " You have not submitted written documentation from the educational institution where you intend to study that states that you have been accepted to study there."The person has not been on the forum since 2019. Better to ask your question.
To clear things up, I am 100% sure that i have uploaded the LOA during my visa lodgement.
Has something like this happened recently?
Did you raise the webform? What was the response? What was written in GCMS Note?
What is your friend’s program, school and start date? So you included LOA, PAL (if required) and paid tuition. My friend program was Construction project management- Fanshawe Toronto (PPP) and the start date was 6th may 2024. However, his visa got delayed and he defered to fanshawe- St. Thomas for sept 2024. Now that he got his visa refused today stating the above reason.What is your friend’s program, school and start date? So you included LOA, PAL (if required) and paid tuition. During initial lodgement PAL was not even introduced (Introduced on 22 jan but visa lodged on jan 11 ). During deferal, the representative told no PAL is required for deferal case and the new LOA so obtained was Added through the webform on may 3 2024 as well.
And yes, tution fee was paid for the first year as well.
So he needs PAL. He changed schools after January 22 and needed PAL for change in schools. Agent was wrong. So he reapplies with new LOA and PAL. Reapply. Webform is for an error in law and should have submitted PAL. Also no timelines for webform and will never know if file is reopened.Is that it? do we have to reapply or can it just be through webform?
Does he even have PAL for the program. Study permit processing is around 14 weeks so if doesn’t have PAL then September intake is unlikely. It doesn’t mean he will be approved as he could be refused for another reason.

He was denied last week Thursday and he applied for his study permit around end of February 2025.
