I am a visitor in canada, I had applied for a job and have been selected. Employer is also giving me LMIA. Which form is needed to be filled for the extension as well as the work permit application.

could you please be specific, it is asking for 5710 only.

So easiest is google - work permit Canada (first Link) and then canada extend visit.
