Already told you. You apply to change the conditions on your work permit. You say you took an IRCC medical so take that medical and number and apply to change conditions. Being a nurse is not relevant.
I am having trouble how do I apply? could you please guide me in that matter?

If you have done the medical in the past year you should be able to change the terms on your visa. There are extra terms of conditions when working with children, patients or elderly people because they are more vulnerable to communicable diseases for example. I order to work in healthcare setting, in daycares/schools, etc. you need proof that you don’t have any health conditions that could be transmitted to the more vulnerable population. It is fairly easy to get rid of the condition
Could you please guide me in getting rid of the condition?

Log into your account. We cannot walk you through this as instructions and guide are there. Hire a consultant to do it for you.
And if I fill out the application for changing the terms and conditions will it be free or will include the charges as they do for extension?
