Hi, Must the Document Checklist (IMM 5488) be submitted with the work permit application? If yes, which section should we attach the form to?
I have forgotten to upload this with my application. Can I send it by web form?

Yes it should be

If you have not then you can attach by webform but it may or may not be acknowledged

Also, Visa Officer may send you an additional document request asking for more info
