Second ID for a kid

Hello ,

For a second ID for my son, I have only the expired OHIP card that has no photo. They are asking for one of the following as a second ID with a photo:

School record
Health insurance card
Hospital or immunization record

I have none of these with photo.

Please advise me.

Usually health card is the best option but since it is expired, the quickest alternative for you might be requesting school for the school records carrying the child’s date of birth.

PR card is accepted only if the other provided ID is also Canadian issued.

We included OHIP along with the minor's passport.
The instruction says that: Both pieces of identification should show the minor’s name and date of birth, one of which must have a photo on it.
So if you are providing existing passport (which will have photograph and other biographical details) along with OHIP it will fulfill the requirement
