Form 5713E for SOWP

Hi

I need a quick suggession. My husband is in Canada and I am applying for SOWP for me and study permit for our son from India. IRCC is asking me to fill form 5713E. Can someone pls confirm what details do i need to fill in this form. Did i selected anything wrong ?
I am using my GCKey to apply for SOWP and Study permit for our son. My husband GCKey is different than mine.

This is a form since you are applying as a family with your son
Make sure you fill out everything right

And attach all the necessary documents to your application

Thank you. But he is minor and as per form only adults can sign the form. Since his age is just 5 years I am not sure how to fill the form. Any guidance on this topic
