Hello...I am applying for citizenship for self as well as kids. for Kids, the ID is required as below. The Personal ID 1 is easy, we have both for kids (PR card as well as Indian passport) but for Personal ID 2, none of the options have a Photo and signature. Any suggestions what to use for Personal ID 2?
Personal identification (ID)

You must provide 2 pieces of ID. Your ID needs to
  • have your name, date of birth, photo and signature
  • be valid (not expired)

Personal ID 1

* Choose the ID you want to provide:
Biographical page (bio page) of the minor’s passport or travel document
Permanent resident card (PR card)
Personal ID 2

The second ID must be issued by the Canadian government.

* Choose the ID you want to provide:
School record
Health insurance card
Hospital or immunization record
Other ID issued by a foreign government

just use the health card. We used health card and no issues. Children do not have any ID with picture(optional on Health card) and signature.

Thank you, yes the Ontario Health Card does not have picture and signature. When did you submit your application? Was it long back? Just thinking if they have changed the process.

You could also get a school record, which will have a photo of your minor child. We got one from Peel.
