Name change in childhood

Hi, question: In the eAPR form it asks me if I've ever used a name before. My older, expired passports (that I've never used to travel) have a different surname, but all of my other documents (including birth certificate, police certificate, current passport, current PGWP, study visa, etc. all have one surname (that is different from the one on my older passports). This is because my surname was changed before I travelled anywhere on my official documents to match my surname on my education documents.

Because I haven't travelled on my older passports, only my changed surname is on file here. Even my birth certificate has the changed surname because we got a new birth certificate made so my name could be consistent across all documents. And it is, all relevant documents have this changed surname - there is no trace of the other surname, save for my older passports, which I don't need to share anyway.

My question is, do I need to answer "yes" to the question "have you used a different name previously", and if so, what documents should I provide? (In my study visa and work permit applications I've said "no" to the answer because I didn't think at the time that this name change in childhood was important/I didn't remember it then.

Thanks for the reply. Review of some other previous posts on the internet showed me that it could be risky if it's found that I used a different name in my childhood and i answer 'no' to this question. Is there any way for them to find out about the previous name? The name was changed due to inconsistency in documents at that time, I don't have anything to hide, but I also don't want the application officer to get confused/delay the application seeing the name change.

Have you applied for immigration to any country with that name? If not then you are ok.

Thanks so much! I have not used that name for any immigration to any other country. On a separate note though, my dad just found the official legal document that certifies my name change (I need to get an official english translation for it).

If I do end up disclosing this name change in the eAPR, will this document, combined with a LOE suffice? Or is there any other required document I need to provide if I disclose the name change?
