it can take upto 3 months to process medical results but can be faster also

The 1st card is normally free only a renewal is $50 per card.

Based on this article from the IRCC help centre I am a new permanent resident of Canada. Do I need to apply for a permanent resident card?

If you cannot create an account within 140 days of the invitation, they will just cancel the free card and you will have to apply for a renewal and pay the 50 bucks on your own.

I think this is unprecedented people normally just wait for the invitation, maybe search this forum and see if anyone was like you and applied b4 invite and see what happened also this IRCC Help centre article Confirming your permanent residence online says that

Do not create your own account. This can delay the process. Follow the instructions in the email we sent and we’ll create it for you.

If you want a better answer call their call centre and see if they can help you, but i think they might just say, they cannot help you, and then they will let you wait so they can get some money, i hope that is not the case..

I did medical for myself and my wife on May 8, but it didn't show in my account and received another medical exam letter on June 11; I called IRCC, and they told me they received medical results on May 17, but the officer didn't update my account and still shows medical results have not been received. I got another medical letter for my son yesterday and another letter today that says the letter was sent on June 11 by mistake, My son's medical also done today and I have to wait for officer again.
