Certainly doesn’t help but will most likely not lead to refusal. You should be updating your marital status with CRA and you application will be amended. You can send in the amended NOA with you correct marital status.

No. Lots of people don't update their status when they should.

I opted outland application and in this scenario there was no requirement to provide NOA because my wife is living with me as well.

Not sure how this is relevant. You could have attached NOA as proof that you support your spouse. Has nothing to do with applying inland or outland. You still need to fix your marital status because you may have received an incorrect amount of benefits.
