Wrong DOB Date of Birth in CSQ

I have received our CSQ in 2015 November. I was postponing my application to CIC because of my current JOB. Now I wanted to apply for CIC. BUt, the date of birth for me and my wife are both incorrect in CSQ. what should I do? I want to add my dependent child who was born after getting CSQ as well. Advices needed.

I had same issue last year. I had filled up my wife's DOB incorrectly in MON. I had mentioned it in cover letter while submitting documents after I had received checklist. I was hoping that MIDI guys will fix this issue in my application and will issue CSQs with correct information but unfortunately it did not happen.

I returned my wife's CSQ along with a cover letter explaining the issue and proof of DOB. I received corrected CSQ within few weeks.

Yes. Cover Letter + All Original CSQs + Proof of DOBs. Please ensure that proof documents are as per MIDI's requirements ( notarized , translated etc. etc.)

Also, call MIDI customer care and ask them if they need anything else.
