If you don't have a surname, then N/A, but if you have a surname, but don't use it, you have to include it.

U should write N/A if not written in your passport. but later You will need to get ur passport updated with ur surname As i have come to know about it recently that if u dont have any surname u do not get social security no.in US and I believe same should go with Canada. So Better to get ur surname added in ur passport.

my brother got SSN in US not sure about canada?
one problem he is facing is that he is not getting the driving license issued without surname.(its abt US?

next q's is if there is no suname in your passport, did u face any problem in getting driving license?

hi my passport got only given name. once i had issue for opening bank account. i don't have USA driving license. its better to change in passport n i am thinking to do so.

are you on H1B Visa?
then how you manage to drive without DL?

coz if u change name in your passport there in US, your visa will be invalid, then you have to again reapply for visa from your native country?
