Would a same name affadavit be required?

Hi all. My name on my passport is let's say A+B while my father's name is C+D. I registered for IELTS with my national ID. In this ID my name is combined with my father's name so on IELTS TRF the name is as first name = A, last name B+C+D.

will an affadavit be needed this case?

PS in my PCC there is also my name combined with my father's name but my passport number is also mentioned in the PCC.

Just a summary:
On passport:
My name = A+B
Father's Name: C+D

On IELTS TRF:
First name: A
Last name: B+C+D

Will an explanation in LOE be enough?

Hey, just for safer side you can get an affidavit for same name. If you search the forum for same name affidavit you can see some samples.

That's the issue. I'm having difficulty obtaining an affadavit.

I can however obtain an affadavit from my home country. Although the national ID I used for ielts is issued from UAE and I sat for IELTS in UAE as well. I am a resident of UAE now. Will it be advisable to get an affidavit from a country other than country or residence?

Hi all. Finally found an oath commissioner who is able to attest my declaration on plain paper.

the only question left is, will notarization/attestation on the plain paper would be fine?
