Hello, please can anyone help with the following three questions:
1. I have a prof corp company and I am employed by the health authority. My salary goes directly to my corp company and I take salary from it. My work involves travelling to different cities and towns but the main health authority is the same. So, in work section, do I have to write my company name as an employer or the health authority's name and if I have to write down health authorities name, it would be 4 different lines (2023-05 till present) in 4 different cities? I am confused about it.
2. Do I have to mention in address proof regarding my quarantine stay of 14 days as well in the list?
3. In education, I am doing a distance learniing course from online university. Do I need to mention that in Education?
