Wife's maiden name and marriage certificate

Hi All - post our marriage our marriage certificate has my last name as her last name.

All her documents has her maiden name and she hasn't changed her name post marriage. Apart from the marriage certificate, everything else has her maiden name including her Canadian visas and Open Work Permit.

She has never marked using another name in any other VISA document, application. Does he need to declare using another name and if so, can she just attach a LoE explaining the same or does she need a same person affidavit?

The name in the app needs to be the name in her passport. If she uses the married name, she can list it an another name. If she doesn't use it, then no need. She certainly doesn't need an affidavit.
