wife dont have family name in her passport

Hi every body

My wife don't have any family name in her passport.
I have question regarding what to fill - "should i leave space blank" or i should write N/A in family name of my wife.

In Generic Application Form [IMM 0008],
- Its not allowing me to enter "N.A" or "N/A" and not even allowing the blank space.
- Should i write 'Given Name' in place of 'Family Name' & keep Given Name Blank or write 'Given Name' at both 'Given Name' and 'Family Name'.

Please Suggest.

NSG.... If your wife does have a family name that is declared everywhere except passport, then have the passport corrected first.
