why does the receipt have a big box in the middle

Hey guys,

i just paid 1040 for the Immigration (paid the 490 residence fee in advance), and in the middle of the sheet it has a big box that says payer information. Am i supposed to fill it up, or does CIC fill it up for me?

Please advice.

I know it seems confusing and there can be conflicting information... But that form I believe says...fill it in and keep a copy and send one w your package. On a side note..I do wish the people putting the information together would cross check w each other. Many things are vague or unclear (e.g medical process vs what happens w the clinic)

You fill in your name and address, send original, and photocopy 1 for yourself . If you can photocopy medicals police reports any thing you can just in case .
