who sign the Schedule A declaration form

CIC requetsed to fill up Schedule A declaration form for Spouse

Question 1) Indicate where you are
the applicant or the spouse
so if she is filling up the form then she has to select the spouse ?

Question 2) who sign at the declaration of Applicant

if she is filling up the form then does she sign or as mentioned applicant signature, i have to sign?

Both the sponsor and the applicant need to do this form. You type your name in the box (electronic signature) and then print and sign by hand
