Who should SIGN on children's forms - CIT0003 & Online Residence Calculator form

have got to post my forms day after. I am submitting my application along with my children's forms CIT0003 as well as Online Residence Calculator form

I have 2 kids age 12 & 8.

To the best of my understanding 'Parents' should sign the forms where it says ---Child's Signature

Is that right?

Yes. From the guide http://www.cic.gc.ca/english/information/applications/guides/CIT0003ETOC.asp:

The parent or legal guardian(s) must sign the application, using the same signature they currently use on other official documents. Enter the place (city, town or village) and date beside the signature.

Sign and date the application on the same day.

Note: Children who are 14 years of age or older must also co-sign the application form with the legal guardian who is applying on behalf of the child.

hi cant you help me also about the parent surname (maiden name if applicable) what do i need to put in there is that my surname when Im single? or i can put my married surname ? any help please tnx

write the same name as it appears on you PR card

hello thanks for the reply but it was really confusing because there's an open and cloce parenthesis maiden name if applicable so iput my maiden name when im single and in other box other name use i put my full name same in my PR card i already submit my application and i dont know iF its right any help pls tnx

To the best of my knowledge the maiden name option is for those who got divorced and still have the same sur-name as they had during their marriage. While I filled up my wife's form I only write her married (after marriage) name. The logic is I don't want to confuse the officer!
