Who needs to fill out IMM 5669?

This is for a spousal sponsorship.

The guide keeps mentioning "your family", but what does that mean in the case of spousal sponsorship? If my wife is the one being sponsored, do her parents and siblings all have to fill out copies of this form? Or perhaps they only need my wife and I to fill it out, since we're also a family of our own.

Only your wife fills it out. It is a form for the sponsored person. The sponsor does not have to fill it out.
CIC defines family as meaning your spouse or common partner or conjugal partner, and any dependent children. They do not mean what is normally meant by family - that is, your wife's parents, grandparents, cousins, siblings, etc. do not fill out a copy of this form, do not need to get medicals done, do not send in immigration photos, etc.
There is one form - Additional Family Information IMM5406 - where parents and all siblings of the sponsored person must be listed. There are a couple of questions on other forms that ask for the names of her parents as well.
