Could you be more specific about the form you're referring to. Does it have an IMM number?

The principal applicant fills out IMM0008.

It's not too complicated. Just check on the CIC website and you will see the 3 sections of IMM forms that must be completed. They are divided into 1) sponsor, 2) applicant, and 3) country specific instructions for the applicant.

How about the minor dependent living outside Canada? Is it required for them to fill-up this form?

All minor dependents (under age 22 and not married or c/l) of the applicant must be included on IMM0008, whether or not they are accompanying the applicant to Canada, and must be examined. Medical exam only if they are under age 18; medical and criminal if they are over 18.
