which one should be on the top? Barcode of IMM0008 or Checklist?

Hi guys,
IMM5612 page one notified that "(the checklist should be) included in your application as the cover page", while as the application guide (IMM 7000) described in the section of IMM0008, "Place the barcode page(s) on the top of your application (forms and supporting documents) ..."
So which one should be on the top? Barcode of IMM0008 or IMM5612-1 Checklist?

In another word, should the IMM5612-1 checklist counted as one of the forms? I'm a programmer so sorry to be too serious and logically... I need your advices.
