Your not looking in the right place l, at : http://www.cbsa-asfc.gc.ca/menu/AFAF_B-e.html there is both the B4 and the B4a.

You need both, 4a is continuation of 4, look in the middle of the B4, you check off if it goods to follow, blank if goods accompanying.

I'm filling out the B4 & B4a forms and on the bottom portion of the B4 form are spaces to write WHERE and WHEN you signed the form. Do I write down the US city where I'm at or where I'm landing, which is Vancouver? Just needed clarification so I can make an extra set of the forms ready for customs.

You sign and date it when you get to Canada Customs.
