Where/how to apply for visitor record extension

Hi,

My wife is a US citizen and has got a visitor record which is valid for 6 months. Now we need to extend it.
I checked the cic website , they have forms only for extend your stay as a WORKER and a form for "STUDENT" but not for VISITOR.
I tried downloading a form but its corrupt and doesn't work : http://www.cic.gc.ca/english/information/applications/guides/5551ETOC.asp

The toll free number is automated and is not helping. Found border service agents who's numbers get forwarded to an ad.

We are ready to go directly to the office to do this extension please. If you think we can do this online any pointers is so much appreciated.
We only have 15days more. Dint expect it to be this confusing.

Have your wife select visit > 6+ months temporary stay > and country of residence Canada on the first page:

http://onlineservices-servicesenligne.cic.gc.ca/eapp/eapp.do

I found this option even while selecting US as the passport-issuing country as OP said his wife is a US citizen.

So either you are doing something terribly wrong or you are looking for something else.

It's for extension of stay.

There's nothing wrong or corrupt with the file: You have to download it to your computer and open it in Adobe Acrobat Reader. Don't open it in your browser.
