What visa office should I use?

Hi, my soon to be hubby and I have decided to do outland, and I'm getting ready to fill out the paperwork. It said on the CIC website that I need to send him my paperwork and he mails it to Mississauga. What what visa office do I put down if I'm from the USA? Buffalo?

Also, when filling out the personal history form, do I do a whole year at a time? Because frankly, I ran out of room at august of '09.

use a separate paper if required make sure you enter the applicants name and form number on the top

No. Be as exact as possible with the dates. You can attach a separate sheet of paper so you can get everything included.

It's supposed to be month/year if I remember correctly. I attached 2 seperate pages, I believe using a table template on Microsoft Works, like they have on the form. Also, make sure you understand it's "10 years OR since age 18, whichever period is longer". I almost made that awful mistake... thinking it was since age 18, but really I had to put down since 2000!
