What should I put as my 'status in Canada'?

My parents immigrated to Canada and became permanent residents. I was two or three years old at the time. I gained citizenship in 1997.

I am confused on what to write as my status in Canada.....?
The options are:
-Canadian citizen by birth
-Canadian citizen by descent
-Naturalized Canadian citizen
-Permanent resident

Can someone explain to me what each of these mean too?

Also, for the last two options a date (YYYY-MM-DD) is required.....and I don't know where to get that? There is only the year listed on my citizenship card. And what is a UCI/Client ID?
