What medical examination documents to enclose?

Hello everyone. I'm quite confused as to what I should enclose in the sponsorship application in regards to medical examination. I received a letter per email that said I completed the examination, but that all my tests were still incomplete. 14 days later and I still haven't heard a peep from them, so I had assumed all I needed to attach to my application was proof that I did the exam. However, there's two different document checklists, one for sponsor (that also includes a checklist for the applicant, that would be me), and then a second one specific for my region.

The first one reads "Proof of medical examination for the sponsored person and each of his or her dependants (Copy 1 of Medical Report: Client Biodate and Summary (IMM 1017), signed and provided by the panel physician). The second one just reads "Proof of medical examination - Include for yourself and each of your family members, proof of completion of the upfront medical examination from the Panel Physician". Going by the second, I'd assume the PDF I received by them in email saying I completed the exam would be enough, but by the first, it sounds as if I am to receive the entire medical report all signed before I can send in my application (IMM 1017??).

Can any of you specify what exactly I should enclose and whether I'm supposed to wait for a completed file to be sent to me before sending off the application? *inhales* Thank you *.*

It looks somewhat similar, though the instructions on it are more vague. I'll take it I should just disregard the first document checklist saying I need a signed document (IMM 1017) and send the receipt!

I sent the receipt as well as the document they emailed to me (same as what you received). Of course, I just sent me application in last month, so I don't have 100% proof that that is what you need, but I'm about 99% sure it is :)

+1. The medical records go to a database. There is a reference number on the form the officer needs to retrieve your full medical information. The physician will not give you a full medical file, just the form as indicated by deepax so the officer can retrieve your information. Once they start the process you will see medicals received on your file when they retrieve your information. Remember that almost of the information is electronic and even the paper forms will be scanned.

Oh, I see. Thank you all! I will just enclose the document I got from them by email (which is what I called receipt, but I suppose it's more right to just call it proof of examination).
