There are two reports you can request:

1) Traveller Passage Report, which shows your entries by name. What is given is your name and the place and time you entered Canada.
2) Highway Passage Report, which shows your entries by licence plate number.

If you want #2, you need to include your licence plate number and proof of your connection to that plate (copy of your car registration).

In our case, there were 92 total entries. 78 appeared on #1, 12 of the 14 missing entries appeared on #2, and two entries didn't appear anywhere.

It depends on who you are, what you ask for and how the particular agent runs the report. If you are something other than a Canadian or US citizen you might get entries to the US included on the report (if you ask for them) for the period after June 2013. Some agents will be more thorough and run the report with variations of your name, others will just run it by document number. Sometimes the information differs depending on what documents you used to enter Canada (or the US).
