What happens if dependent is not able to enter canada before Medical report expiry after successful PR?

Your question doesn't quite make sense. Are you asking what will happen if a dependent can't land by the expiry date of the PR visa? If that's what you're asking, then the answer is that the principal applicant will have to sponsor the dependent at a later date if the dependent still wants to become a PR. Depending on the country, this could take anywhere from a few months to a few years. It will cost additional money and take more paperwork.

"Your medical exam results are valid for 12 months only. If you do not come to Canada as a permanent resident within that time, you may need to have another exam."

This only applies if the PR application is still being processed. If you read the OP's post again, you'll see the question is about an application that has been approved. Once an application is approved and the PR visa is issued, you must land by the expiry date on that visa. You cannot extend the PR visa by completing another medical. It's use it or lose it.

From what I understood, in addition to entering/landing in Canada before the expiry of the PR visa (as indicated in the COPR, which is your point that I totally agree with), you must come to Canada as a PR within the time your Medical Exam is valid or else, you might need to have another medical exam.
