If iam sending the attested transcript in the closed envelope sealed with university stamp to WES by myself, do I have to fill in Academic Records Request Form by the university controller and include it in the sealed envelope or not since i am sending by myself

You can send it by yourself provided all transcripts have been attested & sealed by school.

So the sealed envelope contain only the attested transcripts without the other form at wes Academic transcript request form thing ?
