WES process ! URGENT KINDLY

If iam sending the attested transcript in the closed envelope sealed with university stamp to WES by myself, do I have to fill in Academic Records Request Form by the university controller and include it in the sealed envelope or not since i am sending by myself (EGYPT)

Its not required to attach an Academic Request Form. Attested & sealed transcript is enough. WES is practically useless for any meaningful admission.
