No. It is just a mean to facilitate communication between you and your university. No need to send it.

Only if your university were to demand the form and wouldnt agree to send the transcripts without it.

As long as you get your university to send all the oroper documents to WES or they give them to you in a sealed envelope, you can just ignore the form.
