Is it mandatory to send the 'Academic Transcripts Request Form' along with the official transcripts ? I did not get that form signed from the university as I needed to go to college first, ask for their permission and then ask the university to sign it, which was a lengthy process. So, I got the transcripts without that form, now shall I send just the transcripts or the 'Form' is necessary ?
