WES Evaluation for ACCA Member

Hi All,
I am trying to create my application on WES website. I would like to know about a few things I am stuck on especially the dates.
I started my ACCA in 2011 and completed in 2014.
However I only got my membership this year (2019).
So what dates should enter in the below sections.

Year Awarded: 2019
Years Attended : 2011 - 2014 or 2019

Hey. I am an ACCA affiliate. What did you choose as Country of Education and Institution Type, Name of the Institution and Name of Diploma/Certificate?
