We would feel more comfortable doing this because we have read incidents where, for example, they did not use a representative, did not include the form, and their package was rejected.

2. Also, in the country specific instructions – there are forms for other countries. Does my husband have to fill them out as well? Or only the one specific for Israel.

3. Police certificate – Israel does not provide this. He went down there and they told him that once we file and get a file number he can then apply for the police certificate and it will go directly to the embassy. So we would have nothing to include in the form under police certificate. Is this true? The checklist says it is required in the package.

4. Does the sponsor need a medical? It is on my checklist, we did not notice it until we reprinted the checklist today. Has this changed?

5. Lastly, in country specific instructions; it states for any family member who is Canadian to submit a copy of their citizenship. Does that apply for husbands father?

1. You don't need to include those forms, they should only be included if applicable.
2. Only the principal applicant needs to fill them out - the person being sponsored should do the forms for their own country
3. You can send the police form later, CIC will likely request it. Perhaps include a letter explaining so they don't assume you forgot.
4. Sponsor does not need medical. Are you talking about #21 on the checklist? That's only if you have adopted children. And below that, #27 is the medical exam for the principal applicant (sponsored person)
5. I'm unsure about this one. Usually, family members only refers to spouses and children.
